STEP 1) GET THE FORM

Tip: Use 8.5”x11” paper — check both sides printed!
STEP 2) COLLECT SIGNATURES

Get up to 12 friends, family, or neighbors to sign per form — watch them sign in person, but wait to sign the back "Circulator Affidavit" section in front of a notary (see step 3).

Rules: Registered voters only. Make it readable. You can sign too — just don’t witness a form that lists yourself.
STEP 3) GET IT NOTARIZED

Take your signed form to a bank or notary — then sign it yourself as a witness in front of them (include name, address, date).

Tip: Most banks do it for free — call ahead!
STEP 4) SEND IT BACK TO US
